About Us

CanSPARK LTC is a collaborative initiative bringing together many partners to transform LTC using a collective impact approach. We are fostering a shared vision, to collectively build a continuous learning ecosystem with research, timely data, innovation and coordination of efforts to ensure that all LTC decisions and care are person-centred and evidence-informed.

We are committed to building bi-directional partnerships, inclusive of all interested partners including public and private long-term care homes, residents, families, care partners, staff, associations, organizations, researchers, and governments.

Vision

For Canada to be a world leader in providing evidence informed person-centered care and wellbeing for those who live and work in LTC.

Mission

To drive an innovative pan-Canadian Learning Health System Model for continuous improvement in LTC using a Collective Impact Approach to achieve the Quintuple Aim.

Goal

To enhance person-centered care and quality of life through research, evidence, data, innovation and collaboration of LTC homes, residents, caregivers, and all partners.
